Your Mac and the screen stay on until you press again.
Your screen doesn’t go black in the middle of a talk.
A pop-up tells you each time whether Caffeine just went on or off. Closing the lid still puts the Mac to sleep, and in Low Power Mode on battery macOS has the last word.
